You are invited to JSC's SAIC/Safety and Mission Assurance (S&MA) Speaker Forum featuringBryan O'Connor, Former NASA's Chief of Safety & Mission Assurance. 

Mr. O'Connor will share and discuss the following points:  1) Lessons from human spaceflight mishaps; 2) Importance of open communications in managing high risk activities; and 3) Value to the leader of good checks and balance.

Bryan O'Connorretired as Chief of Safety and Mission Assurance on August 31, 2011, after serving nearly a decade as NASA's top safety and mission assurance official. O'Connor is a former Marine Corps test pilot and aeronautical engineer, with more than 5,000 hours of flying time in over 40 types of aircraft. He joined the NASA astronaut program in 1980 and flew two space shuttle missions, serving as pilot on STS-61B in 1985 and commander of STS-40 in 1991.

O'Connor is an aerospace safety consultant and joined the ASAP in May, 2012.  The Aerospace Safety Advisory Panel (ASAP) was established in 1968 and has been evaluating NASA's safety performance and advising the Agency on ways to improve that performance. The ASAP bases its advice on direct observation of NASA operations and decision-making. In the aftermath of the Shuttle Columbia accident, Congress required that the ASAP submit an annual report to the NASA Administrator and to Congress. The annual report is to examine NASA's compliance with the recommendations of the Columbia Accident Investigation Board (CAIB), as well as NASA's management and culture related to safety.
