Look at Apollo, shuttle, as time went by & more got killed, safety became a bigger concern. Apollo, shuttle had considerable funding as compared to some of the underfunded new starts.
After a fatality accident, accident board will convene, big delay, rework, and then a few flights, and another accident, hence the cycle repeats,
Unless we get competent leadership, doubtful we can regain Preeminence in space!!!
